An exciting opportunity to be part of the new national network of Link Workers, working with the Chanctonbury Central Primary Care Network Group covering the Billingshurst Surgery and the Glebe Surgery in Storrington.

This is a new innovative role and the successful candidate will work as part of a multi-disciplinary team in a patient facing role, designed to work in an outcome focused way to improve people’s quality of life, health and wellbeing by recognising that this can be affected by a range of social, economic and environmental factors.

Supporting patients and their carers to achieve their personal aspirations participate in their local and wider communities, enhance effective personal support networks, enabling individuals to maintain healthy lifestyles; and lead independent and fulfilled lives.
