Carers Health Team

We are a team of health professionals who aim to promote the health and wellbeing of carers. We are a free service provided by Sussex Community NHS Trust providing support for carers over the age of 18 living in West Sussex. We work directly with carers and will develop individual strategies for each carer with an aim to reduce the strain of coping with their caring role. We may if it is necessary also look at the health needs of the person who is being cared for.

We work closely with other NHS specialist services, social & caring services and voluntary & community services and will link carers with these services when appropriate.

We can provide information and advice on specific health conditions, as well as practical advice on the caring environment (safe lifting and handling, healthy eating, risk minimisation)

We will visit carers in their own home at a time to suit their needs.

Health Visiting (West Sussex)

Health visiting is a universal ill health prevention and health promotion service offered to all families with children under the age of five years in their own homes and in community settings such as children’s centres.

Health visiting teams lead and deliver the Healthy Child Programme (HCP), providing services covering pregnancy and the first five years of life. The HCP is the evidence-based public health programme for children and young people, which provides a range of health interventions and support beginning in pregnancy and continuing through early childhood. Health visitors can play a crucial role in ensuring that families get good, joined up support at the start of life. A health visitor can, for example, provide clinical intervention to a depressed mother struggling with a new baby; or identify a child’s speech or language problem that would benefit from early help. These are just two examples of many areas where health visitors can provide support and information to help parents to access available services.
